Life Insurance

  • Provides a cash benefit to your loved ones in the event of your death • Features group rates for Madison County Commission employees.

  • Includes LifeKeys® services, which provide access to counseling, financial, and legal support services.

  • Also includes TravelConnect® services, which give you and your family access to emergency medical assistance when you’re on a trip 100+ miles from home.

Local Government Health / Dental Insurance Plan

  • Offered to eligible employees and their eligible dependents. LGHIP is administered by Blue Cross and Blue Shield of Alabama using a national PPO network.

  • Includes dental and prescription coverage at no additional premium.

Bi-Monthly Employee Premiums

 

Single Coverage: $59.00

 

Family Coverage: $143.50

Vision Services

Plan

  • Eligible employees may voluntarily enroll in VSP for eye care insurance. This plan allows employees to receive eye exams, lenses, and frames.

  • Dependent children may be covered on Vision through age 26.

Bi-Weekly Premiums:

 

Single: $2.77

 

Employee + One: $5.82

 

Family: $8.92

Flexible Spending Accounts

  • Enrollment allows an employee to pay for medical expenses or daycare services with pre-tax dollars.

  • Employees must enroll annually.

  • Madison County offers two voluntary flexible spending plans (flex plans) to eligible employees for the 2023 plan year:

Health Care: $3,050 limit

Dependent Care: $5,000 limit

Teladoc gives you convenient access to a doctor through LGHIP for FREE in 10 minutes or less using video call through phone or computer.

Teladoc can help!

24/7 access to doctors, including pediatricians. They can prescribe treatment for non-emergency conditions like the flu, allergies, bronchitis and more.

Employee Clinic

Enjoy a $15 copay at your Madison County Employee Health Clinic. Available to employees, retirees, & their dependents over age 12 on Madison County's health insurance plan.

Prescription Drug
Benefits

Madison County Employees enrolled in the health insurance plan can enjoy discounts and reimbursement on select drugs. Provided by OptumRX, the Prescription Drug List (Formulary) includes the covered drugs available for the plan. Drugs not included in the formulary are not covered without prior authorization from OptumRx

Expectant Mother & Child Care

Baby Yourself

BCBS's free maternity management program provides education, support services, and nurse consultations. Provider copays and hospital deductibles are waived for participants

Enrollment must be before the third trimester

 

Immunizations and Routine Well Child Visits

Covered at 100%, no deductible or copay

 

Allergy Testing and Treatment

Covered at 80%, subject to the calendar year deductible

 

ABA Therapy

$14 copay/visit for children on the Autism Spectrum, precertification

is required
